Things you should bring with you the day of your appointment:
- X-rays, MRI's or Cat Scan films
- A list of medications & dosages you are currently taking
- Insurance ID card & social security number (yours & your spouse's, if covered by additional insurance they carry)
- Worker's Compensation paperwork, claim information, adjuster name and claim number
- Co-pays are due at time of service
